Heure - Time

Lieu (en cas de lieu-dit, indiquer la commune) -The commune and locality in effect.

Département - Department( the first two numbers of the post code).

Espèce présumée - The species of bird you think it is

Conditions et circonstances de découverte de l'oiseaux (éventuellement, état de fraîcheur du cadavre). - In what conditions or circumstances did you find the bird, how decomposed etc.

Vos nom et adresse - Your name and address.

Observations complémentaires - Any other observations

Try if possible to identify the species.
